I Am Boundless in Ohio is hiring a Health Data Specialist to gather, monitor, and report outcomes data for the OhioRISE CME program. This role may work remotely or from an office location, collaborating with care coordination and management teams via phone, video, and email.

The specialist will monitor a large population served by the organization’s care coordination and management activities and ensure all required reports are complete, coordinating with care coordinators to close data gaps as

Qualifications

• High School or GED required.
• 1–3 years of experience with health data, data analytics, health outcomes reporting, or data reporting tools.
• Valid Ohio Driver’s License with auto insurance; no more than 5 points.

Responsibilities

• Gather, monitor, and report outcomes data for OhioRISE CME.
• Monitor a large population supported by care coordination and care management activities.
• Ensure reporting is complete and address data gaps with care coordinators.
• Interact with staff from other provider or community organizations as needed.
