You will monitor a large population of individuals and families in need, address data gaps with the care coordinator, and collaborate with external partners as needed.

Qualifications

• High School or GED required.
• 1–3 years of experience with health data, data analytics, health outcomes reporting, or data reporting tools.

Responsibilities

• Gather, monitor and report outcomes data for the OhioRISE CME.
• Monitor a large population of individuals and families in care coordination and management.
• Collaborate with care coordination teams via phone, video, and electronic communication to address gaps.
