CVS Health is seeking a Telephonic Registered Nurse to support patients across the country from a remote home-based setting. You will educate patients on medication adherence, assess needs, and contribute to care plans while communicating with clinicians.

The role emphasizes strong communication, advanced computer skills, and a focus on safety and quality within a nationally distributed team. A state license and multi-state privileges are required.

Qualifications

• A Registered Nurse must hold an unrestricted license in their state of residence, with multi-state/compact privileges and be able to license in non-compact states as needed.
• 3+ years of clinical RN experience required; 5 years preferred.
• Experience using Microsoft Office, including Word, Excel and Outlook.

Responsabilités

• Provide education and support for patients' new medications via outbound calls.
• Develop, implement, and evaluate comprehensive care plans for multiple disease states.
• Collaborate with health care professionals and the Compliance/Persistency teams to ensure continuity of care.
