Job summary

Alignment Health is seeking a Health Coach & Educator to conduct home and virtual visits, building a wellness coaching relationship with patients, caregivers, and families to improve health through support, education and encouragement.

You will collaborate with clinicians and social workers, deliver health education materials, and help patients access resources to address social determinants of health, while maintaining HIPAA privacy and providing outstanding patient service.

Qualifications

• Must be able to communicate positively, professionally and effectively with patients, caregivers and families.
• Knowledge of diseases, nutrition, exercise, and behavior change models.
• Ability to work independently and remotely while maintaining confidentiality under HIPAA.
• Strong interpersonal skills and customer service focus.
• Bilingual skills preferred but not required.

Responsibilities

• Contacts patients eligible for Alignment's Care at Home program and provides wellness coaching.
• Collaborates with clinicians and social workers to develop action plans.
• Encourages adoption of healthy habits to improve quality of life.
• Delivers high level patient service and supports member retention.
• Assists patients in accessing community resources addressing social determinants of health.
• Develops and distributes health education materials to patients and families.
• Coordinates care with Alignment team members and external vendors.
