Qualifications

• Board Certified Health and Wellness Coach.
• Proficient with computers, virtual care and data tracking.
• Knowledge of Motivational Interviewing and SMART goals.
• 2+ years of health coaching experience.
• Experience with digital care delivery or telehealth.
• Excellent written and spoken communication.
• Empathy and motivation for patients.
• Proficient with Microsoft Office.
• Reliable high speed internet and dedicated home office.
• Ability to work in a fast-paced startup environment.
• Strong time management and collaboration skills.
• Commitment to learning and feedback.
• Nice-to-haves: Bi-lingual Spanish.

Responsibilities

• Provide remote health coaching to support patient goals.
• Use Motivational Interviewing and SMART goals in care plans.
• Deliver video consultations and secure messaging with patients.
• Document interactions and track data for care coordination.
• Collaborate with inter-disciplinary teams to optimize care.
• Maintain professional knowledge and participate in education activities.
