Job summary

CVS Health is seeking a Health Advocate to join the Healthy Outcomes Team. You will engage with Medicare members telephonically to promote healthy behaviors, explain benefits for screenings and medications, and connect them with services and resources to improve Star health outcomes.

You will talk to providers and members to aid with important health services, vaccinations, and medication refills, while documenting interactions in the health record and maintaining a high standard of service.

Qualifications

• Excellent communication and interpersonal skills.
• Strong active listening and relationship-building.
• High computer literacy and ability to troubleshoot issues.
• Proficiency with Windows and web-based tools.
• Reliable high-speed internet and a suitable remote workspace.
• Wired connectivity required (no DSL/satellite/wireless only).

Responsibilities

• Handle inbound and outbound calls to members and providers for healthcare services, screenings, vaccinations, or medications.
• Identify and help remove barriers to members' health and access to care.
• Deliver empathetic, professional conversations to ensure the best member experience.
• Educate members to make informed health decisions and maximize benefits.
• Document conversations accurately in the member's health record.
