Job summary

Comagine Health is seeking an HCBS Assessor to perform in-home assessments across the Albuquerque, NM region for adults under the Mi Via Waiver. The role blends field visits with documentation in our electronic systems.

You will collect data on daily living skills, document findings clearly, and support participants in maintaining independence and community access. Travel using a personal vehicle is required with a largely remote setup.

Qualifications

• Associates degree in social work, psychology, other behavioral health science field.
• 2 years of social service, nursing or other relevant work experience.
• Valid, unrestricted driver license, proof of auto insurance and reliable transportation; able to pass Motor Vehicle Record check.

Responsibilities

• Manage your schedule to complete in-home assessments across the New Mexico region using your personal vehicle.
• Coordinate and schedule annual in-home assessments including outreach and follow-up calls and maintain an organized calendar.
• Conduct person-centered, face-to-face assessments in participants' homes, including adults with intellectual and developmental disabilities and/or caregivers.
• Navigate and use multiple technology platforms to document findings, complete templates, upload assessments, and close cases timely.
• Ensure confidentiality and HIPAA compliance, safeguarding participant information and following procedures.
• Communicate with supervisor and team to report on work and discuss cases; participate in training and meetings.
