Job summary
Total Life Inc. is launching a telehealth Longevity Program focused on comprehensive geriatric assessment, chronic disease management, cognitive health, and GLP-1 therapy for adults 65+.
You’ll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth, working with our geriatrician CMO and receiving full training and clinical support. Responsibilities include cognitive screening, medication review, advance care planning, and documentation in our EHR for
Qualifications
- Active NP license and board certification in geriatrics required.
- DEA registration and Medicare documentation experience are essential.
- Telehealth delivery and secure EHR documentation must be routine.
- Private, HIPAA-compliant workspace and reliable internet required.
Responsibilities
- Deliver initial longevity assessments and follow-up care via telehealth.
- Conduct comprehensive geriatric assessments, cognitive screening, and care planning.
- Review medications and manage chronic conditions; adjust GLP-1 therapy per protocol.
- Document in EHR for Medicare billing and care continuity.
- Perform advance care planning conversations and coordinate with the geriatric team.
