Job summary

Total Life Inc. is a telehealth company focused on older adults, expanding a Longevity Program with geriatric assessment and GLP-1 therapy.

You will deliver comprehensive initial assessments and ongoing medical follow-ups for seniors via telehealth, working with our geriatrician CMO and receiving full training and clinical support. Responsibilities include cognitive screening, chronic disease management, advance care planning, and Medicare billing documentation.

Qualifications

• Active, unrestricted NP license in California.
• Board certification as an AGNP/AGPCNP or Family NP with geriatric focus.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.

Responsibilities

• Conduct initial longevity assessments and follow-up visits via telehealth.
• Perform comprehensive geriatric assessments, cognitive screening, and medication reviews.
• Manage GLP-1 therapy for appropriate patients.
• Lead advance care planning conversations when indicated.
• Document encounters accurately in the EHR for Medicare billing.

About Total Life

Total Life is a telehealth company dedicated to older adults. We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.

We're launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy.

The role

You’ll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You’ll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.

What you’ll do

• Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
• Provide medical follow-up visits for chronic disease management and ongoing care
• Perform cognitive screening and staging; escalate for further assessment where indicated
• Prescribe and manage GLP-1 therapy for appropriate patients
• Conduct advance care planning conversations
• Document in our EHR to support accurate Medicare billing

Requirements

• Active, unrestricted NP license in California
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
• Active DEA registration
• Experience with Medicare documentation and E/M coding
• Comfort with telehealth and EHR documentation
• Reliable high-speed internet and a private, HIPAA-compliant workspace

Nice to have

• Experience with comprehensive geriatric assessment
• Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
• GLP-1 prescribing experience
• Multi-state licensure
