Job summary

Total Life, a telehealth company serving adults 65+, is seeking a Nurse Practitioner to deliver initial longevity assessments and ongoing medical follow-ups to older adults. You will conduct geriatric assessments, cognitive screening, medication review, and advance care planning, with GLP-1 therapy management and Medicare billing support.

Work with our geriatrician CMO, with full training provided. This 100% remote role offers flexible scheduling and meaningful work in aging care.

Qualifications

Active, unrestricted NP license in Florida.
Board certification in AGNP/AGPCNP or Family NP with geriatric focus.
Active DEA registration.
Experience with Medicare documentation and E/M coding.
Telehealth and EHR documentation experience.
Reliable high-speed internet and a private, HIPAA-compliant workspace.

Responsibilities

Conduct comprehensive initial longevity assessments (60 min) via telehealth, covering functional status, cognition, nutrition, mobility, and fall risk.
Provide medical follow-up visits for chronic disease management and ongoing care.
Perform cognitive screening and staging; escalate for further assessment where indicated.
Prescribe and manage GLP-1 therapy for appropriate patients.
Conduct advance care planning conversations.
Document in our EHR to support accurate Medicare billing.

About Total Life

Total Life is a telehealth company dedicated to older adults. We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.

We’re launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy.

The role

You’ll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You’ll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.
