We are launching a Longevity Program delivering comprehensive geriatric assessment and chronic disease management with GLP-1 therapy. As a nurse practitioner, you will conduct initial longevity assessments, cognitive screening, medication review, and advance care planning via telehealth, with full training and clinical support from our geriatrician CMO.

Qualifications

• Active, unrestricted NP license in Texas.
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus.
• Active DEA registration; Medicare documentation and E/M coding experience.

Responsibilities

• Conduct initial longevity assessments and follow-up visits via telehealth.
• Provide medical follow-up for chronic disease management and ongoing care.
• Perform cognitive screening and staging; escalate as needed.
• Prescribe and manage GLP-1 therapy for appropriate patients.
• Conduct advance care planning conversations.
• Document in our EHR to support Medicare billing.

About Total Life

Total Life is a telehealth company dedicated to older adults. We provide behavioral health, geriatric care, and longevity services to adults 65+, billed through Medicare, Railroad Medicare, and major commercial insurers. Our network includes 200+ credentialed providers serving patients across 49 states.

We're launching a new Longevity Program focused on healthy aging — comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management including GLP-1 therapy.

The role

You’ll deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth. This includes comprehensive geriatric assessment, cognitive screening, medication review, advance care planning, and management of chronic conditions. You’ll work alongside our CMO, a board-certified geriatrician, with full training and clinical support provided.

What you’ll do

• Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
• Provide medical follow-up visits for chronic disease management and ongoing care
• Perform cognitive screening and staging; elevate for further assessment where indicated
• Prescribe and manage GLP-1 therapy for appropriate patients
• Conduct advance care planning conversations
• Document in our EHR to support accurate Medicare billing

Requirements

• Active, unrestricted NP license in Texas
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
• Active DEA registration
• Experience with Medicare documentation and E/M codingComfort with telehealth and EHR documentation
• Reliable high-speed internet and a private, HIPAA-compliant workspace

Nice to have

• Experience with comprehensive geriatric assessment
• Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
• GLP-1 prescribing experience
• Multi-state licensure
