Qualifications

• Active Registered Nurse license in the state of employment.
• Minimum two years of direct care experience with elders or adults with disabilities.
• Ability to travel within assigned geographic area; valid driver’s license and insurance.

Responsibilities

• Conduct home visits to assess safety and health status; review medications.
• Provide ongoing coaching to caregivers via app; refer resources and reinforce best practices.
• Build trusting relationships to influence positive changes and reduce caregiver burnout.
• Document and review records for accuracy and timeliness per regulatory standards.
