Job summary

Holon Health seeks a Community Relationship Manager to grow partnerships with community organizations, healthcare providers, health plans, and justice system partners in Nebraska. You will educate stakeholders on Holon’s care model and help deliver outcomes that improve access to care.

You’ll be a strategic, relationship-driven leader who mentors the Client Acquisition team, travels regionally, and champions equity and compassionate, human-centered care.

Qualifications

• Bachelor’s degree or equivalent professional experience.
• Experience in account management, community engagement, outreach, or sales—ideally in healthcare, behavioral health, or criminal justice sectors.
• Demonstrated success managing teams and driving results.
• Proficiency in CRM tools and virtual collaboration platforms.
• Experience with community organizations, healthcare providers, health plans, or justice system partners is a plus.
• Ability and willingness to travel throughout Nebraska.

Responsibilities

• Build relationships with referral partners across sober living homes, treatment providers, courts, and community organizations.
• Serve as primary contact for referral partners in your territory.
• Conduct in-person visits, presentations, and trainings to strengthen engagement.
• Represent Holon at local/state conferences and community events.
• Identify opportunities to co-host or join educational/community initiatives.
• Act as a visible ambassador for Holon’s mission and model of care.
• Track industry trends and provide feedback to inform outreach strategy.
• Foster an equity-driven team culture through training and mentoring.
• Other duties as assigned.
