Holon Health is seeking a mission-aligned Community Relationship Manager to grow partnerships across Iowa. You’ll engage courts, community organizations, health plans, and providers to demonstrate Holon’s innovative care model and expand access to care.

You’ll mentor the Client Acquisition team, develop trainings for justice and community partners, and drive regional growth in a remote-first culture. This role combines relationship-building with strategic outreach and hands-on collaboration.

Competenze

• Relationship-building with communities and partners across health and justice sectors.
• Strong speaker with ability to lead trainings and presentations.
• Organized and able to manage multiple outreach efforts and timelines.
• Commitment to equity, inclusion, and expanding access to care.
• Willingness to travel across Iowa as needed.

Mansioni

• Build relationships with referral partners including sober living homes, treatment providers, courts, and community organizations.
• Serve as primary contact for referral partners in your territory and coordinate engagement.
• Conduct in-person visits, presentations, and trainings to deepen partnerships.
• Represent Holon Health at conferences, coalitions, and community events.
• Collaborate with teams to refine outreach strategies based on trends and feedback.
• Mentor Client Acquisition team and develop partner training programs.
• Foster an inclusive, equity-driven team culture through mentoring and example.
