Job summary

Doist is seeking a Nurse Liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate treatment, rehab, and return to work. This remote, field-based role requires regional travel up to 2 hours.

Duties include following the CM plan, accepting referrals, scheduling assessments, maintaining ongoing communication, documenting data, attending physician appointments, and supporting RTW planning.

Qualifications

• RN license current and unrestricted in applicable states.
• Licensed in each state where field case management is provided.
• Ability to perform independent assessments within scope.
• National certifications within 4 years of hire preferred (e.g., CCM, CRC, CLNC, CRRN).
• Two years FTE direct case management for injured workers or two years under supervision preferred.

Responsibilities

• Coordinate CM plan and maintain accountability for the CM process.
• Accept referrals as assigned by the Director of Case Management.
• Schedule initial assessments and obtain date of next physician appointment.
• Maintain ongoing communication with Injured Worker, Insured, and Carrier.
• Record data and billing; submit monthly reports.
• Attend physician appointments to obtain diagnosis, prognosis, treatment plan, rehab length, RTW details.
• Refer to Vocational Counselor for job analyses when appropriate and approved.
• Coordinate transportation as needed.
