Qualifications

• Must reside in Illinois.
• Willing to travel up to 75% in Metro East/Southern Illinois.
• 3–5 years of clinical practical experience.
• Comfort working from home and collaborating virtually.
• Strong analytical and problem-solving abilities.
• Effective communication and interpersonal skills.
• Ability to work independently.
• Proficient in standard software including MS Word, Excel, Outlook, PowerPoint.

Responsibilities

• Collaborate to inform the case management process and assist with appointment scheduling.
• Help members access benefits and education using care management tools.
• Provide support to enable healthcare outcomes at home and in the community.
• Travel to patient locations around Metro East/Southern IL.
