Job summary

NK Consultancy LLC is seeking a Field Nurse (RN) Case Manager to cover the North Chicago/WI border areas. This field-based role requires an active RN license, prior case management experience, and a willingness to coordinate care across employers, providers, and insurers.

The position offers a competitive salary with incentives, mileage reimbursement, and comprehensive benefits. Travel within 1–2 hours of home is expected to support injured workers’ recovery and safe return to work.

Qualifications

• Active RN license required.
• Prior experience in case management required.
• Case Management certification a plus
• Multilingual proficiency is a plus but not required.

Responsibilities

• Clinical Advocacy: assess injured workers' medical conditions and develop coordinated treatment plans.
• Care Coordination: attend medical appointments to ensure continuity of care and alignment with the treatment plan.
• Collaborative Planning: partner with employers to implement job duty modifications based on medical restrictions.
• Communication: serve as the primary liaison between injured workers, medical professionals, employers, and insurance representatives.
• Education: interpret and explain medical instructions to injured workers to ensure understanding.
• Reporting: prepare detailed monthly progress reports and document activities.
