Job summary

UnitedHealth Group in Texas is seeking a dedicated Registered Nurse to join our community health team in the Houston area. You will assess, plan, and implement person-centered strategies, connecting members with resources and coordinating care across the continuum.

This role requires 2+ years in healthcare, an unrestricted Texas RN license, reliable transportation, and the ability to travel up to 50% within the Houston region.

Qualifications

Current unrestricted Texas RN license.
2+ years of experience in a community health or healthcare setting.
Intermediate level of proficiency with Microsoft Office applications.
Reliable transportation, a valid driver's license, and ability to travel within assigned territory.
High-speed internet at residence.

Responsibilities

Assess, plan, and implement person-centered strategies.
Connect members with financial, psychosocial, community, and state resources.
Coordinate care across the continuum and serve as a key point of contact.
Advocate for members and families to ensure their needs and preferences are heard and supported.
Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
Travel 50% of the time within the Houston area and surrounding areas.
