Job summary

Optum is seeking a Field-Based Behavioral Health Care Advocate to work in the Toledo, OH area, largely from home with about 35% field time. You will visit members in homes or other settings, provide education, coordinate resources, and support transitions from facilities back to home.

The role requires an independent license or RN with 3+ years in behavioral health, plus strong computer skills and reliable transportation for local travel. Remote work with travel within Toledo is expected.

Qualifications

Independently licensed clinician with a Master’s degree in Psychology, Social Work, Counseling, or related behavioral health discipline, OR an RN with 3+ years of behavioral health experience.
Licenses must be independent, active, and unrestricted in the state of Ohio.
2+ years of post-licensure experience in a related mental health environment.
Proficient computer skills and good working knowledge of Microsoft Word and Outlook.
Access to high-speed internet and a dedicated, distraction-free workspace at home.
Reliable transportation and ability to travel within the Toledo, OH area.
Reside in or near Toledo, OH.

Responsibilities

Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or provider offices.
Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction.
Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels.
Assist members with the transition from a care facility back to their home.
