Job summary

You will collaborate with healthcare teams to ensure smooth transitions from facilities to home and provide ongoing support for better health outcomes.

Qualifications

• Licenses must be independent, active, and unrestricted in Ohio.
• 2+ years of post-licensure experience in a related mental health environment.
• Proficient computer skills including Word and Outlook; reliable home workspace and internet.
• Reliable transportation and ability to travel within Toledo, OH area.

Responsibilities

• Visit Medicaid members in their homes or other settings.
• Provide continuum of quality care through in-person or phone contact.
• Support members with education, medication reviews, and connections to resources.
• Assist with transition from care facility back to home.

Education

Licensed Mental Health Clinician with a Master's degree (psychology, social work, counseling or related)

Registered nurse (RN) with psychiatric specialty certification

RN with master's in nursing (psychiatric/mental health) and 2 years supervised experience
