Job summary

Advocate Health Care is seeking a senior leader to oversee Oncology Nurse Navigation across its Midwest and Southeast divisions. The role includes enterprise responsibility, travel to support teams, and potential relocation assistance for eligible candidates.

Remote work is allowed for approved locations with travel approximately 2–3 times per month. Minimum requirements include a Bachelor’s in Nursing, 5 years of oncology experience, and OCN credentialing, with Master’s in a related field

Qualifications

Bachelor's Degree in Nursing required.
Master's Degree in Nursing, Healthcare Administration or Business Administration preferred.
5 years of clinical experience in Oncology required.
Experience as an Oncology Nurse Navigator preferred.
Credentialed as an Oncology Certified Nurse (OCN).
Director of Oncology Nurse Navigation/Patient Navigation experience preferred.

Responsibilities

Leads development of standards and methodology to identify patients at the appropriate point within their cancer journey to receive care coordination services via the Oncology Patient Navigator (OPN).
Develops standards for follow up with patients by the OPN and transition points to survivorship programs or end of life care.
Oversees and implements system-wide processes to assess needs and provide access to proper resources for cancer patients with seamless transfers.
Collaborates with staff to design and implement performance improvement monitoring and annual reviews of oncology nurse navigators.
Recommends educational content and facilitates the system-wide Patient Navigator Academy.
