Job summary

Form Health is a fully remote W-2 opportunity for an ABIM-endorsed Endocrinologist with obesity medicine expertise. You will provide evidence-based care for obesity and diabetes via telemedicine, often coordinating with APPs and dietitians to support complex patients.

The role emphasizes collaboration, advanced diabetes management, CGM/insulin pump integration, and a patient-centered approach in a flexible schedule for 32, 36, or 40 hours per week.

Qualifications

• Must have an MD or DO degree.
• Valid and current Board certification in Endocrinology by ABIM.
• Active and unrestricted Medical State License in any US State, with eligibility to obtain additional licenses.

Responsibilities

• Provide direct patient care via telemedicine for obesity and diabetes.
• Collaborate with APPs and Registered Dietitians.
• Review charts and manage CKD/ESRD and CAD/CV conditions.
