Job summary

Form Health is seeking ABIM-certified Endocrinologists with obesity medicine expertise to join our fully remote, W‑2 telemedicine team. The role emphasizes diabetes management, CGMs, insulin pumps, and collaboration with Endocrine APPs and Registered Dietitians to treat Type 2 diabetes and obesity.

This position offers flexible schedules (32, 36, or 40 hours/week), comprehensive benefits starting day one, a 210k–250k USD salary range, and opportunities to mentor colleagues.

Qualifications

MD/DO with ABIM Endocrinology board certification.
Active US medical license and ability to obtain licenses in multiple states.
Experience in obesity medicine or endocrinology with pharmacotherapy.
Comfort with telemedicine and collaboration with APPs.

Responsibilities

Provide direct patient care via telemedicine for obesity and diabetes management.
Collaborate with Endocrine APPs and Registered Dietitians in care delivery.
Review charts, manage CKD/ESRD/CAD-CV conditions, and adjust therapies.
Mentor colleagues and contribute to quality improvement in the program.
