Job summary

Form Health is a remote, telemedicine clinic seeking board-certified Endocrinologists with obesity medicine expertise to lead multidisciplinary diabetes and obesity management. The role emphasizes complex case management, collaborative APP supervision, and a flexible remote schedule (32 or 36 hours/week).

Licensure in NY/VA/MA preferred. Ideal candidates hold ABIM Endocrinology certification and ABOM or extensive obesity pharmacotherapy experience, with strong communication and teaching skills

Qualifications

• Board certification in Endocrinology by ABIM.
• Active and unrestricted Medical State License in any US State; NY/VA/MA preferred.
• LOQ through IMLC or eligible to obtain one upon hire.
• ABOM certification or extensive obesity medicine experience (≥3 years).
• Expertise in CGMs, insulin pumps, and complex diabetes management.

Responsibilities

• Provide direct telemedicine care for obesity and diabetes, including insulin titration and CGMs.
• Lead collaboration with Endocrine-Specialized APPs and Registered Dietitians.
• Review charts and visits for APP patients to ensure quality and CKD/ESRD/CAD management.
• Develop advanced treatment plans for complex endocrinology cases and oncology of obesity.
• Prescribe non-controlled medications for obesity and diabetes.
• Lead weekly 30-minute collaborative meetings with APPs and review cases.
• Ensure adherence to protocols and thorough clinical documentation.
• Support professional development, mentorship, and quality improvement initiatives.
• Participate in team-based care initiatives and department meetings.
