UnitedHealth Group’s UnitedHealthcare division is seeking a Telecommute RN to improve community health outcomes in Texas. The role supports members by linking them to financial, psychosocial, community, and state resources while coordinating care across the continuum.

Travel up to 75% within the Mission, TX area and surrounding regions is expected. The role offers telecommuting with flexibility and a comprehensive benefits package, plus incentives and a 401k plan.

Formación

• Current unrestricted Texas RN license.
• 2+ years of experience in a community health or healthcare setting.
• Intermediate proficiency with Microsoft Office applications.
• High-speed internet at residence.
• Access to reliable transportation and valid US driver’s license with ability to travel within assigned territory.

Responsabilidades

• Assess, plan, and implement person-centered strategies.
• Connect members with financial, psychosocial, community, and state resources.
• Coordinate care across the continuum and serve as a key point of contact.
• Advocate for members and families to ensure their needs and preferences are heard and supported.
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes.
• Travel up to 75% of the time within the Mission, TX area and surrounding areas.
