Qualifications

• Licensed Bachelor’s Social Worker (LBSW) or higher-level license.

Responsibilities

• Monitors and manages care coordination initiatives.
• Assists provider networks and healthcare community toward integrated care models.
• Follows NCQA guidelines for Complex Case Management.
• Maintains a caseload of DWIHN members.
• Performs required documentation in MHWIN promptly.
• Visits members in hospital or community settings.
• Completes assessments and care plans within required timeframes.
• Coordinates with community providers and member care teams.
• Provides transitions of care support after hospital discharge.
• Attends weekly and other required meetings.
