Qualifications

• Undergraduate degree in Nursing degree
• Current, unrestricted Multistate Registered Nurse (RN) license
• 4+ years of diverse clinical experience caring for acutely ill patients with multiple disease conditions; knowledge of utilization management, quality improvement, and discharge planning
• Preferred ED experience; managed care and/or case management experience

Responsibilities

• Monitor real-time ADT notifications to provide proactive outreach and discharge coordination.
• Support ED discharge follow-up using clinical guidelines to transition care to the next site with outpatient resources.
