Job summary

Kelsey-Seybold Clinic in Houston is seeking an ED RN Case Manager to monitor ADT notifications, coordinate discharges, and ensure smooth transitions of care for acutely ill patients. You will collaborate with hospitals, physicians, and outpatient resources to maintain high-quality outcomes.

Qualifications

• Undergraduate degree in Nursing.
• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience caring for acutely ill patients with multiple disease conditions.

Responsibilities

• Monitor real-time ADT notifications to provide proactive patient and provider outreach and discharge coordination to support optimal transitions of care.
• Support ED discharge follow-up care using approved clinical guidelines to transition patients to appropriate next sites of care in collaboration with hospital/physician teams and outpatient resources.
