Qualifications

• Undergraduate degree in Nursing.
• Current, unrestricted Multistate Registered Nurse (RN) license.
• 4+ years of diverse clinical experience with acutely ill patients and multiple conditions.

Responsibilities

• Monitors real-time ADT notifications to provide proactive patient and provider outreach.
• Supports ED discharge coordination and transitions of care.
• Collaborates with hospitals, physicians and outpatient resources to ensure continuity of care.
