Job summary

Kelsey-Seybold Clinic, part of the Optum family, seeks an ED RN Case Manager to monitor ADT alerts and coordinate care transitions for acutely ill patients. The role offers remote flexibility for Pearland, TX, with opportunities to collaborate across hospital and outpatient teams.

The ideal candidate has a nursing degree, an active RN license, and 4+ years of clinical experience, including discharge planning and utilization management. ED experience and case management exposure are a plus.

Qualifications

• Undergraduate degree in Nursing.
• Current, unrestricted Multistate RN license.
• 4+ years of diverse clinical experience with acutely ill patients; knowledge of utilization management, quality improvement, and discharge planning.

Responsibilities

• Monitor real-time ADT notifications to provide proactive patient outreach and discharge coordination.
• Support ED discharge follow-up care in collaboration with hospital/physician teams and outpatient resources.
• Coordinate transitions of care to appropriate next site of care (PCP, specialist, in-home care).
