Qualifications

• Bachelor's degree or higher from an accredited college or university in Education, Social Work, Human Services, Behavioral Science, or a health-related field.

Responsibilities

• Conducts intake interview with family and completes required documentation; offers provider options for families.
• Participates in IFSP meetings to determine child eligibility and functional outcomes.
• Manages and monitors financial and medical services for enrolled children.
• Informs families of rights and available community resources.
• Informs families about State programs such as KidCare.
• Issues accurate authorizations to EI providers for all services.
• Maintains caseload lists and regular client communication in Cornerstone.
• Attends Case Review meetings with Medical and Social Emotional Consultants.
• Initiates transition from EI to school services as required by DHS.
• Distributes documents to team and attends LIC/AOK meetings as appropriate.
• Fixes authorizations and assists in data corrections in Cornerstone.
• Conducts developmental screenings on an as-needed basis.
