Cunningham Children’s Home is seeking a compassionate, organized, and collaborative Integrated Care Coordinator to join our Community Services team. This position plays a key role in supporting youth and families in Champaign County who are at risk of becoming involved in the juvenile justice system.
The Integrated Care Coordinator coordinates services through the Families Stronger Together (FST) and Juvenile Redeploy Illinois (JRI) programs, working closely with youth, families, schools, court services, attorneys, community providers, and other partners to promote stability, strengthen families, and help youth remain safely connected to their communities.
The core schedule is Monday–Friday, 8:30 a.m.–5:00 p.m., with flexibility required to meet the needs of youth and families. This position may require up to three evenings per week and occasional weekends.
