Cunningham Children’s Home seeks an Integrated Care Coordinator to join our Community Services team in Champaign County. The role supports youth and families at risk of juvenile justice involvement, coordinating referrals, services, and individualized plans.
You’ll work with youth, families, schools, court services, attorneys, and community providers using a strengths-based, trauma-informed, youth-driven approach to promote stability and safe community connections.
Cunningham Children’s Home is seeking a compassionate, organized, and collaborative Integrated Care Coordinator to join our Community Services team. This position plays a key role in supporting youth and families in Champaign County who are at risk of becoming involved in the juvenile justice system.
The Integrated Care Coordinator coordinates services through the Families Stronger Together (FST) and Juvenile Redeploy Illinois (JRI) programs, working closely with youth, families, schools, court services, attorneys, community providers, and other partners to promote stability, strengthen families, and help youth remain safely connected to their communities.
Salary is based on the experience. We are committed to the values of spirituality, teamwork, integrity and respect. Prospective employees will be required to submit to a pre-employment physical and background check.
Bachelors or better in Human Services or related field.
Masters or better in Human Services or related field.
Child welfare, work with emotionally and/or behaviorally disturbed or developmentally delayed children/adolescents, and/or families.
