Job summary

Actalent in the United States is seeking a Case Management Assistant to support discharge planning for a large hospital system. This role focuses on assessing post-hospital needs, coordinating referrals and services, and documenting in Epic EHR, Allscripts, and other systems.

You will communicate clearly with patients, families, providers, and payers, manage high-volume data entry, and help ensure timely transitions of care in a fast-paced remote environment.

Qualifications

• High school diploma or equivalent required.
• EPIC EMR experience required.
• 1 year of healthcare experience.
• Typing 55+ WPM.
• Knowledge of HIPAA regulations.
• Strong data entry skills.
• Proficient in Microsoft Office.
• Excellent patient and payer communication.

Responsibilities

• Assist discharge planning with Case Managers to ensure safe transitions.
• Assess post-hospital needs and coordinate referrals and authorizations.
• Maintain Epic EHR and Allscripts documentation accurately.
• Communicate discharge timelines to patients, families, and providers.
• Process high volumes of faxes, voicemails, and data entry.
• Support utilization review and transportation coordination.
