Qualifications

• BSN with current, unrestricted RN license
• 5+ years of recent acute inpatient hospital experience
• 2+ years discharge planning or case management
• Experience with Medicaid/Medicare/Commercial products in the insurance industry
• Proficient typing and MS Office suite
• Dedicated home workspace with secure high-speed internet

Responsibilities

• Evaluate discharge needs and readmission prevention plans
• Coordinate post-discharge care with providers and members
• Participate in rounds with Medical Director to discuss cases
• Identify internal or community resources for support
• Coordinate with Discharge Planner to arrange post-hospital services
• Document discharge activities per SOPs and data-entry
• Participate in team meetings and educational discussions
• Collaborate with care teams to ensure seamless end-to-end care
