Job summary

Avardis Health Management seeks a Corporate Director of Social Services to provide strategic leadership and operational oversight for Social Services across our skilled nursing and rehabilitation centers. This leader will partner with regional and center teams to strengthen regulatory compliance, resident and family experience, care transitions, behavioral health support, and quality outcomes.

Qualifications

• Master's degree in Social Work.
• Active Social Worker license where required by state regulations.
• At least five years of Social Services leadership experience in skilled nursing or long-term care.
• Extensive knowledge of CMS regulations and requirements affecting Social Services.
• Strong expertise in discharge planning, care transitions, resident rights, behavioral health, and psychosocial services.
• Ability and willingness to travel approximately 50–75%, including overnight travel and occasional extended stays.

Responsibilities

• Develop and standardize Social Services processes, policies, tools, and documentation across Avardis facilities.
• Lead admissions support, psychosocial assessments, resident adjustment, discharge planning, and care transitions.
• Support CMS requirements, Social Services-related F-Tags, resident rights, abuse prevention, PASRR, care planning, and behavioral health.
• Coach Social Services Directors through onboarding, competency validation, education, performance improvement.
• Drive resident satisfaction, family engagement, and quality-of-life initiatives.
• Monitor readmissions, grievances, satisfaction, and survey trends.
• Lead QAPI initiatives and use data to identify improvement opportunities.
• Partner with Operations, Nursing, Therapy, Admissions, Business Office, HR, and providers for seamless journeys.
• Provide on-site support during vacancies, surveys, transitions, and high-risk situations; interim facility role when needed.
