Job summary

Avardis Health is seeking a qualified Director of Social Services to lead our social services department at Locust Grove Retirement Village. You will oversee resident and family support, admissions assistance, and the referral process, ensuring compassionate, compliant service and a positive facility culture.

The role requires a degree in Social Work or related field, prior long-term care experience, and strong regulatory knowledge.

Qualifications

• Bachelor's or Master's degree in Social Work, Human Services, or related field.
• Experience in long-term care, healthcare, or social services within a skilled nursing or similar setting.
• Strong understanding of state and federal regulations governing social services in nursing facilities.
• Excellent interpersonal, communication, and problem‑solving skills.
• Ability to build and maintain relationships with residents, families, and external agencies.
• Compassionate, dedicated, and committed to enhancing the lives of residents.

Responsibilities

• Stay up to date with federal and state regulations and advise on policy changes.
• Implement policies addressing residents' social and emotional needs.
• Engage in community planning to support facility services.
• Oversee discharge planning and development of social care plans.
• Provide consultation to staff and external agencies for social programs.
• Develop corrective action plans for social service deficiencies after surveys.
• Maintain up-to-date reference libraries of laws and standards.
• Conduct and document social services evaluations identifying resident needs.
• Maintain communication with case workers and external providers.
• Provide social work consultation to residents, families, staff.
• Drive innovation to improve social services and outcomes.
