Avardis Health is seeking an experienced Corporate Director of Social Services to provide strategic leadership, operational oversight, and field support for Social Services across skilled nursing and rehab centers. This role partners with regional teams to strengthen regulatory compliance, resident experience, and care transitions.

Qualifications

• Master's degree in Social Work and active licensure as required by state regulations.
• Minimum five years of leadership in Social Services within skilled nursing or long-term care.
• Strong knowledge of CMS regulations and requirements affecting Social Services.

Responsibilities

• Develop and standardize Social Services processes, policies, tools, and documentation across Avardis facilities.
• Guide admissions support, psychosocial assessments, resident adjustment, discharge planning, care transitions, grievance management, and family communication.
• Provide leadership on CMS requirements, Social Services-related F-Tags, resident rights, abuse prevention, PASRR, care planning, behavioral health, and trauma-informed care.
• Support survey preparation, complaint investigations, Plans of Correction, regulatory recovery efforts, and IDR/IIDR activities when appropriate.
• Coach and mentor facility Social Services Directors through onboarding, competency validation, education, performance improvement, and career development.
• Champion resident satisfaction, family engagement, resident and family councils, behavioral health programming, and quality-of-life initiatives.
• Monitor performance indicators including readmissions, successful community discharges, grievances, satisfaction, care-plan compliance, behavioral health outcomes, and survey trends.
• Lead Social Services-related QAPI initiatives and use data to identify and sustain improvement opportunities.
• Partner with Operations, Nursing, Therapy, Admissions, Business Office, Human Resources, and medical providers to create a seamless resident journey.
• Provide on-site support during leadership vacancies, surveys, transitions, high-risk situations, and operational improvement efforts; serve in an interim facility role when needed.
