AEIC Case Management LLC is seeking providers already approved or contracted with Texas HHSC for CPW to join our independent contractor roster. This remote position offers visits conducted within your approved scope and does not guarantee referrals, hours, or income.

You will complete comprehensive visits, assess needs, and coordinate services while adhering to AEIC documentation standards and program requirements.

Qualifications

• You must maintain timely, accurate documentation.
• Strong organization and communication skills are essential.
• Current HHSC CPW provider approval or contracting status is required.

Responsibilities

• Complete comprehensive case management visits within your approved scope.
• Assess needs and develop service plans; coordinate resources and follow up on progress.
• Maintain timely, accurate documentation and protect client confidentiality.
• Follow AEIC documentation standards and payer requirements.
