• 2+ years of acute care experience required.
• Knowledge of Medicare/Medicaid regulations and utilization management processes.

Responsibilities

• Develops complex discharge plans based on level of care required by needs assessment.
• Interacts with providers and other staff to coordinate safe transitions between care facilities.
• Manages discharge plans to ensure timely, smooth transitions and appropriate care levels.
• Documents all discharge plans in health management systems per policy.
• Educates members and families on discharge plans and utilization processes.
• Provides feedback to leadership to improve discharge planning processes.
• Assists with prior authorizations related to discharge planning.
