Job summary

Centene Corporation is seeking a Registered Nurse to develop and coordinate complex discharge plans for high-risk members, ensuring smooth transitions from acute care to next level of care. This remote role requires an active compact RN license and experience in case management or discharge planning in a telephonic setting.

Responsibilities include documenting discharge plans, coordinating with providers and community agencies, and educating members and families about the plan and utilization

Qualifications

• Requires Graduate from an Accredited School of Nursing or Bachelor’s degree in Nursing with 2–4 years of related experience.
• 2+ years of acute care experience required.

Responsibilities

• Develops complex discharge plans based on level of care determined by needs assessment.
• Interacts with providers, community agencies, and other utilization review staff to coordinate transitions.
• Assists with managing discharge plans to ensure timely and smooth discharge between levels of care.
• Documents discharge plans in health management systems.
• Educates members and families on discharge plan and utilization processes.
• Provides feedback to leadership on improving discharge planning processes.
• Coordinates completion of prior authorizations related to discharge planning.
• Assists concurrent review team as needed.
• Performs other duties as assigned.
• Complies with policies and standards.
