Job summary

Tuesday Health is seeking a licensed practical nurse to join our Dayton-based team delivering coordinated, compassionate care to members with serious illness. The LPN will work with nurse practitioners and RNs to conduct assessments, support care plan execution, and help ensure a seamless member experience.

This role emphasizes relationship building, clear communication, and adaptability to evolving care needs.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical/medical settings preferred.
• Ability to multitask and adapt to evolving care plans.

Responsibilities

• Administer assessments and screenings with NP/RN support.
• Prioritize needs and contribute to care plans.
• Develop and execute multidisciplinary care plans.
• Participate in rounds to coordinate member experience.
• Learn and use Tuesday Health electronic tools.
• Build trusting relationships with members through empathy.
• Serve as the quarterback of the care team for complex cases.
