Qualifications

• RN license in New York (current, unrestricted).
• Care coordination or telehealth/triage experience preferred.
• Experience with chronic disease management (CHF, diabetes, HTN).
• Proficiency with EHRs and standard office tools; telehealth tech comfort.

Responsibilities

• Serve as a remote care‑coordination resource for HV North sites.
• Identify discharges within 24–48 hours and initiate post‑discharge outreach.
• Conduct telephone medication reconciliation and renewals per SRH protocols.
• Integrate patient assessments with site primary care teams and communicate via EHR.
