Responsibilities

• Connect members to known community resources to support their care management journey.

• Provide non-clinical health assessments to refer members to appropriate care/services and to gather information for providers.
• Conduct telephonic and/or in-person outreach to locate individuals and families in the community who are hard to reach.
• Support the clinical team with outreach, community education, informal guidance and member support.
• Document and maintain all community resources to ensure standards of practice and policies are followed.
