Job summary

Abound Health is seeking a Community Liaison & Business Development Manager to join our Growth team. This remote role requires up to 50% travel across Pennsylvania and focuses on expanding referral networks, generating member leads, and supporting intake across IDD, Pediatric PDN, and Personal Care lines.

You will build partnerships with schools, agencies, and community organizations, manage communications, and provide ongoing partner updates to ensure alignment with care coordination.

Qualifications

• Bachelor's degree required or equivalent experience in a relevant field.
• Experience in healthcare, human services, or IDD/PDN programs preferred.
• Strong relationship-building, communication, and presentation skills.

Responsibilities

• Identify, cultivate, and maintain relationships with referral sources such as schools, agencies, and community partners.
• Schedule and conduct in-person visits, presentations, and follow-ups with partner organizations.
• Attend & host community events to promote Abound Health across IDD, Pediatric PDN, and Personal Care markets.
• Generate new client referrals and warm leads; ensure prompt communication and handoff to intake and care coordination teams.
• Track referral activity, conversion rates, outcomes, and trends using OnTarget, Microsoft Dynamics, and PowerBI.
• Provide ongoing support to referral partners with performance updates and issue resolution.
• Ensure full compliance with ethical referral practices and healthcare regulations.
