Job summary

Stormont Vail Health seeks a Community Health Worker (CHW) to outreach to community members, guiding them through the medical system and linking to services in clinic and community settings. You will educate patients, coordinate care, and support access to resources.

You will build trusting, confidential relationships, document encounters in the EMR, and collaborate with the inter-disciplinary team to address social determinants of health and facilitate timely follow-up.

Qualifications

• 1 year healthcare-related field experience.
• Knowledge of prenatal care, breastfeeding, infant and child development.
• Confidentiality and sensitivity in care.
• Ability to work independently and with multidisciplinary teams.
• Excellent time management and organizational skills.

Responsibilities

• Provide coordination of care to patients and their families.
• Provide education and support to patients and families.
• Provide social determinants of health screenings, referrals for services and appropriate follow up.
• Build and establish trusting relationships with patients, healthcare providers, agencies and other resources to connect patients to services.
• Assist families with enrollment into programs and resources (e.g., KanCare, Medication Assistance).
• Follow up with patients regarding appointments/referrals, identify barriers to care and help solve them.
• Document all appointments, referrals, and encounters in the EMR.
• Network with other agencies and programs to ensure awareness of available services.
