Job summary

Bernalillo County seeks a Community Case Manager in Albuquerque to provide case management and counseling to assigned clients across residential, community-based, and jail-based settings. MINIMUM QUALIFICATIONS include a Bachelor’s degree in Criminal Justice, Psychology, Sociology, or a related field and one year of related work

Qualifications

• Bachelor’s degree in Criminal Justice, Psychology, Sociology, or a related field.
• One (1) year of work experience in criminal justice, social work, counseling, or a related field.
• LSAA preferred.

Responsibilities

• Conducts initial client interviews and gathers case histories to assess needs and care levels.
• Coordinates with community agencies to schedule treatment and support programs.
• Supports reliability and effectiveness of client treatment and services.
• Provides case management by identifying needs and referrals for treatment and services.
• Establishes and maintains relationships with community partners and professionals.
• Collaborates with families, justice system personnel, and resources to support progress.
• Transports clients to appointments and service providers.
• Maintains accurate and timely case documentation.
• Prepares case files for regular review and quality assurance.
