Job summary

Ladders is seeking a Remote Clinical Program and Population Health Strategist to lead government program initiatives. The role focuses on shaping population health strategies for ACA, Medicaid, and D-SNP populations, driving outcomes for complex, vulnerable groups, and aligning with CMS requirements.

The candidate will translate strategy into provider-facing actions, collaborate across clinical, operational, and data teams, and advance health equity in all programs.

Qualifications

• Bachelor's degree required.
• Master's degree (MPH, MHA, MBA, or related) preferred.
• 5-10+ years of experience in population health, healthcare strategy, or clinical program design.
• Experience with Medicare (D-SNP) and/or Medicaid programs.
• Strong understanding of HEDIS, Stars, risk adjustment, and utilization drivers.
• Experience working in a matrixed organization.
• Proven strategic thinking and portfolio prioritization abilities.
• Proficiency in data-driven decision making and cross-functional collaboration.

Job description

For our client, we are seeking a Remote Clinical Program and Population Health Strategist-Government Programs to join the team of a leader in the Hospitals & Medical Centers space.

This role will shape population health strategy for government program members, with a focus on improving outcomes for complex and vulnerable populations. The strategist will help design and refine clinical initiatives, translate strategy into actionable provider-facing approaches, and ensure alignment with regulatory and program requirements. This position will work closely across clinical, operational, and data-driven teams to prioritize the highest-impact opportunities and advance health equity.

Location: Remote

Compensation: $108,680 - $190,694 annually
