PRN Clinical Pharmacist

Job summary

Thrifty White Pharmacy is seeking a Pharmacist for a remote role. You will deliver MTM services, including Comprehensive Medication Reviews (CMR) and Targeted Medication Reviews (TMR), and collaborate with prescribers to optimize therapy.

Responsibilities include evaluating indications, efficacy, safety, and adherence; proposing cost-effective interventions; educating patients; and documenting outcomes. Requires Minnesota license and ambulatory care experience; remote position with flexible

Qualifications

• Must hold Minnesota pharmacist license (and ND/IA licenses as applicable)
• Valid CPR/BCLS certification
• Immunizations certification required

Responsibilities

• Deliver MTM services including CMR, TMR, and disease state management to optimize therapy
• Use patient care process to deliver care and address medication needs
• Determine interventions to resolve therapy problems and prevent new ones
• Use evidence-based guidelines to formulate recommendations
• Assess indication, efficacy, safety and adherence for each regimen
• Assist with cost-effective interventions including formulary considerations
• Assess prescribed therapy for unnecessary meds, inappropriate doses, and interactions
• Evaluate and document clinical responses and adverse effects
• Conduct patient-centered adherence services and address barriers
• Collaborate with prescribers and care teams to set treatment goals
• Communicate recommendations to prescribers and teams
• Provide patient education on chronic diseases and therapies
• Maintain knowledge of CMS and PQA measures
• Meet productivity and performance expectations
