Hillcrest Health & Living is seeking a Clinical Intake Specialist for Hillcrest Home Care. This role reviews referrals, coordinates admissions, verifies insurance, and ensures compliant handoffs while maintaining patient confidentiality.

A flexible, remote option is available for the right candidate, with Bellevue, NE-based applicants preferred. This is a full-time position starting at 32+ hours per week and offers a collaborative team environment that supports patient transitions.

Qualifications

• Active RN or LPN license in Nebraska or compact state.
• Two+ years in healthcare, including acute/post-acute.
• Experience with home health admissions, intake, or referrals preferred.

Responsibilities

• Review referrals for clinical, admission, and financial criteria.
• Complete clinical reviews and determine placement per guidelines.
• Communicate with hospitals, discharge planners, providers, and families.
• Build relationships with referral partners and provide timely information.
• Screen referrals and gather required clinical information.
• Enter and maintain referral data in CRM/EHR systems.
• Verify insurance coverage, eligibility, and authorizations.
• Explain insurance details to patients and families.
• Complete pre-admission and handoff to Home Care for start of care.
• Coordinate with hospital discharge planners for transitions home.
• Monitor referrals and follow up on missing information.
• Improve referral conversion and census goals with the team.
• Maintain referral-management and census reports; provide backup support.
• Educate partners about Hillcrest services and care continuum.
• Maintain HIPAA confidentiality.
